add template for requesting access to credentials store #1654

This formalizes a process for requesting to be added to the shared vault for a given repository.

what would be the process after one of these issues is submitted? do you need them to ping you on slack to give you their email address? maybe we should have them include their slack username in the request so you can verify its them that is reaching out? are you the only person who can handle these requests currently? thx!

are you the only person who can handle these requests currently?

I thought all the GC members were already able to add users, but I just looked and realized they weren't "owners" on the account. I've changed those settings just now.

Ideally I think the best folks to be owners of the account would either be GC or TC members. WDYT?

@open-telemetry/technical-committee 1password feels maybe most similar to github org ownership, do you want to own it?

Reaching out on slack might be the best option as i don't think we'd want people to put their email addresses directly in the github issue... would be great if there was a way to use their github handle instead
