Switch to using Create Facility Form

Currently, we have two forms (old one called: FacilityCreate, for which we had a recent tickets to add support for editing the facility, including supporting Organization Selector).

Tasks

• Add support for editing existing facility (similar to functionality in FacilityCreate)
• Add support for organization selector Facility edit Form Does Not Prepopulate State, District, Local Body, and Ward Fields #9648 (most of the logic is already done in Edit feature for facilities in organization | pincode, geo_organization info auto populates #9662)
• Pincode exception Pincode Field Auto-Populates but Throws “Expected String, Received Number” Error on Submission - When editing and existing facility #9649
• Rename CreateFacilityForm -> FacilityForm
