Facility edit Form Does Not Prepopulate State, District, Local Body, and Ward Fields

When editing an existing facility, the edit form does not prepopulate the fields for State, District, Local Body, and Ward. As a result, the user is required to re-enter these details manually to submit the form successfully.

Expected Behavior:
The edit form should prepopulate the State, District, Local Body, and Ward fields with the existing values

@Jacobjeevan SearchByMultipleFields is only used in patients page, what about other pages? 🤔
