Open a binary file to read instructions fro that file. This one uses mmap to open the file read only.
This also adds an equality operator for the Instruction type.
This change removed the production on/off distinction in the build process, instead using command line options to run the tests, execute a bytecode file, or display a help message. This means that tests are always compiled into the final binary.
