Tools/Items from Anvil or EnchantingTable are disappear sometimes a few Moments later.

Expected result: Rename/repair/enchant Tools and they are able to use it.
Actual result: The renamed/repaired or enchanted Items sometimes dissapears a few Moments later or are crafted back to the original Components.

Steps to reproduce the issue

Rename/repair or enchant Tools at Anvil or EnchantingTable.
Try to use it, change InventorySlot or try to put them in a chest.
Sometimes they are useable and sometimes they will dissapear

OS and versions

Altay: 3.11.8
PHP:7.2
Server OS: Debian 10
