Subscription option is not appearing on the comment form.

I've confirmed the StRC settings, however, when I check the comment section, the option to subscribe and be notified when there is a follow-up comment is not showing up.

Sorry for the wait.

The issue is that your theme has CSS code that hides label elements in the comment reply area.

Probably added to hide the labels on the name,email, comment fields, but that had the unintended effect of hiding all labels including the one that our plugin adds.

Add this CSS code in WP admin > Appearance > Customize > Additional CSS:

.comment-form-subscriptions label { display: block !important; }
