Cannot make reservation on GLPI 9.5.7 with Addressing Version 2.9.1

I can add a new item as reservation. After I click "Validate the reservation" button, no information are stored in the plugin and no error is visible. Everything is blank.

How can I solve this issue? The settings are default, I changed nothing.

Is there no way to reproduce and solve this issue for GLPI 9.5.x ???
