I haven't looked but I assume the optional descriptive text for images is included in the API response. If so, pull that out and use it in templates etc as alt text?

As of March 2020 there are no plans to add the alt text to the API response: https://twitterdevfeedback.uservoice.com/forums/921790-twitter-developer-labs/suggestions/38653933-alt-text-via-api
