Describe the bug
A checkbox symbol is near the Back button.

To Reproduce
Steps to reproduce the behavior:

  1. Select a District
  2. Click on "View Facility"
  3. The checkbox icon displayed next to the button "Back."

Expected behavior
The Back button should display a left arrow instead of a checkbox.
