Resource request form is not submitting and resources are not displayed properly in board view

Resources request form is not submitting and Resources are not displayed properly in board view

I have debugged and tried to create resources and the form is not submitting due to sub category error

And also the created resources are not dispalyed properly in board, they should be visible according to status

@Jacobjeevan @rithviknishad I have done some of the changes in ResourceCreation form. Please assign this to me if it is relevant. I will work on it

Steps to reproduce:

1. Go to encounter page and click any patient
   2 Next click on patient details page
   3 Clock on request tab and create a request
