Readmission of "Telemedicine" Patient to the same facility is not showing proper discharge reason

Describe the bug

A telemedicine patient from the facility is readmitted as a regular patient to the same facility, and the discharge reason is displayed as "Unknown.

To Reproduce
Steps to reproduce the behavior:

1. Create a patient by marking it as telemedicine while creating the new consultation
2. Now, readmit the same patient as a normal patient (without marking telemedicine in consultation) to the same facility through the patient registration page
3. once it is successfully created, go to patient detail page and check the discharge reason of the telemedicine consultation page it will be shown unknown "https://care.coronasafe.in/facility/08da6228-0911-4108-ae77-9461015cee87/patient/dd4614b2-e74d-4e55-8307-537e1418b52d"
4. See error

Expected behavior

if they are trying for transfer to same facility, throw an error saying that there is already a consultation present for this patent
