Add options on the copied value

Hi All.

I'm new on this plugin but i have a challenge. I want that along with the copied value to be able to insert 3 inputs where i can store some values. (or select boxes)

From what i can see in the demos when you click on the copied value it's going back. I need to be able to enter the values as i present earlier.

Do you have an example like this?

Let me give you an example.
So in the left side, i have some options, when i click on an option it's "moving" to the other side. Till here all perfect. But i also need to add 3 inputs where a user can add some numbers (like scores 1-5).
Can anyone show me an example or point me in the right direction?
