Questions do not get filtered

When adding questions that contain media, the media does not get filtered and shown correctly. For example,

• I have a multiple choice question with a mp3 file. The file name is shown instead of the media player, along with the question text and answer text.
• I have a multiple choice question with a picture and text. The picture is missing but the text and answers are shown.
• I have a question with a Generico filter that adds the username into the question to personalize the question. The plain embedded filter code is shown instead of the user name along with the rest of the question text.
