Duplicate buttons in toolbar

What steps will reproduce the problem?
1. Install NiftyPerforce-1.3.9.msi
2. Open Visual Studio.

What is the expected output? What do you see instead?

There are now some buttons duplicated in the Niffty toolbar (show revision 
history, show time lapse view, show revision graph).  The second button in each 
case does nothing when clicked.

I have attempted to reinstall niffy, but the issue continues to occur.

What version of the product are you using? On what operating system?

* NiftyPerforce-1.3.9
* Visual Studio 2010
* P4V 2011.1/405081
* Windows 7
