This integration breaks after every reboot (sometimes after every HA restart, too). Each time it requires a new token to be set up and when it works again several devices aren't found and have to be set up again.
I can confirm this behavior. For me, it even started with 2024.12.5.
However, I believe this project has no maintainer since at least one year, so I doubt there will be any solution soon.
I can confirm I am experiencing the same issue recently.
Since updating to the most current versions of HAOS and HA, if HAOS host is restarted Smartthings Integration is completely removed from Home Assistant.
Upon restart Smartthings is Discovered but not configured. Re-adding the integration with a new personal access token works well. I am willing to do any testing or logs and have subscribed to this issue. I see the original report is sparse on details:
