The audio should come through the speakerphone

Audio came out through the earpiece.

Strangely the speakerphone is listed under 'microphone'.

On my phone Samsung A54 5G with Android 14 the call always starts playing through the speaker phone instead of earpiece so it's kind of reversed.
Also speaker phone / earpiece options are under microphone rather than speaker and I can confirm this part.
