Set ctsi_study_id in service_instance when we get novel values from CSBT

We need to set ctsi_study_id in service_instance when we get novel values from CSBT. update_invoice_line_items_with_invoicing_details.R could be doing this, but it us not. It or some other script should set that value when we learn novel ctsi_study_ids.

Should we add the complexity to update_invoice_line_items_with_invoicing_details.R or move it somewhere else? Related to this, we might soon need another novel process to groom the data in that same table. See Sharing CTSI Study IDs in issue #143. We could put both processes in a novel script and run it nightly to keep the data clean.
