Ensure all value sets in the valueset directory are expanded

By default, the HAPI server is set to require expansions for value sets while evaluating CQL. This is the most performant option, so should be supported by default for testing scenarios, but some of the value sets in the vocabulary folder in this repository don't have expansions. Make sure that all the value sets have expansions.
