Accessibility Feedback Loop

Thought I'd start this as a discussion and see what people think of it.
But the idea is creating an Accessibility Feedback Loop for the Codú site.

Reason:

I can appreciate that accessibility is a spectrum, and there's only so much people can consider when coding.
But why not let those with accessibility needs share their experience and feedback on how to improve on it?

Process concept:

1. High level overview of the idea is to create a feedback form that would email @NiallJoeMaher.
2. Niall would then determine if there's sufficient feedback on the form to create a new issue
3. Not enough feedback? Potentially collaborate through email discord etc... on how to improve the site.
4. Yes - sufficient feedback? Create issue on Github with the feedback
5. Issue then gets assigned and closed off as per usual process.

If anyone has a better idea on a process - please by all means. It's just a concept for now.

Expectations:

I don't want people to think that this gives them a "free pass" on accessibility, but if something has been missed, overlooked, or a perspective that just was not considered, at the very least there is an option for people to share their feedback.
