Create a custom issue template for User Story #4

As we are writing user stories, it would be nice to have an issue template that had that info prepopulated.

Steps:

Copy the folder structure found in our website custom template (this includes the path!).
Copy the template linked above, and rename it something like user_story.md.
Change the copied template with the content we need (ask @alliequintano about the content, and ask @novellac if you need help with the markdown).
Make a pull request (PR) and assign @novellac as a reviewer.

Acceptance criteria:
When I click the "New issue" button on Github, I should see an available template.
