Allow user to retry/review puzzles #78

According to feedback on reddit, people don't like not being able to retry puzzles. The simplest way of dealing with this is only counting a user's first attempt at a puzzle when calculating ratings.

The other option would be to allow a user to review a puzzle, to explain where they went wrong and potentially why. This would (maybe) require permanently blacklisting puzzles, but this would be fine if we can generate puzzles using #16.
