Now that CSD3 uses TOTP for login, would be good to include instructions about this.
Need to go to the extension settings and in "Remote SSH: Show Login Terminal" tick "Always reveal the SSH login terminal".

This will enable interactive login from a terminal tab that opens within VS Code.
