feat: Add options to customize what header information to be included into log

As of now, the middleware can not be configured to add specific custom header information into the produced log. Sometimes one need to make custom log which include these information due to some enterprise logging/tracing requirement.

I've made an enhancement to this great library that provide this feature #5. If this help, it will be great if the changes can be merged here.
