Reload on SIGHUP

Send a SIGHUP to the rq-mgr process to pick up new queues and relay the SIGHUP to all running queues. Many other small cleanups.

Usable Open Source Release

This is our first release of RQ since becoming open source! This release includes many code cleanups, testing on Travis CI, improved documentation in Markdown, and an improved init script.
