Manual title with version doesn't display with jupyter-book version 0.14 and higher #218

The title and version that were displayed in the upper left corner of the docs page is missing when compiling the manual with jupyter-book >=0.14.0

I've noticed a number of projects have pinned 0.13.2 so it would be worthwhile to look if there is an issue we can track.
