To Reproduce
Steps to reproduce the behavior:
Install this version of Chisel & bits:
chisels-and-bits-fabric-1.3.135.jar
(any other for 1.19.2 causes the same issue)

any other mods I used where fine. I created an installation with just fabric + chisel & bits and taterzen.
Also occurred in a modpack using performance mods such as sodium etc.
Once the game started, create a new world, or use an existing one.
Port away from the spawn point so it´s no longer rendered. I tested it with porting to x 1111 y 64 z 1111 in a freshly created world.
Change the spawnpoint /setworldspawn
Kill yourself with /kill
Then create an npc, enter equipment mode. It won´t be able to equip with anything except shields. Nothing is working in the main hand. Shield can be swapped though.

NOTE
As long as the spawn point is not changed it seems to work someone. Once however the spawn point is changed it´s broken. Also directly applies to worlds created with e.g. worldpainter where the spawn point is changed.

Can you provide some logs.
Maybe there is some evidence inside of them.
