Generate docs for builders

Currently builders are undocumented.

If a doc comment for the constructor exists and it contains an arguments section then copy the docs across.

The type signature problem needs to be solved otherwise docs look awful, but I don't remember why an intermediate typedef for the builder state was not used. Need to revisit.
