loadfacts include entries for FHIR:Resource.id, FHIR:Meta.lastUpdated, FHIR.Meta.versionId, FHIR:Narrative.status, FHIR:Reference.display, FHIR:Reference.reference, etc.

generate_i2b2 does not generate concept / modifier dimension entries for these codes.

We need to decide what does and does not belong and implement them consistently.

Note: test_concept_coverage.py is how this is currently detected.
